Pregnant women who report a penicillin allergy can safely undergo penicillin skin testing and incremental drug challenges without adverse effects in their pregnancy, according to a study published in Annals of Allergy, Asthma & Immunology.

The need for delabeling

Beta-lactam antibiotics are first-line treatment for preterm pre-labor rupture of membranes , chorioamnionitis and prophylaxis for cesarean deliveries among other conditions in pregnancy, according to the researchers. Although approximately 10% of the population report a penicillin allergy, the researchers continued, more than 90% of patients with reported penicillin allergy can safely tolerate it after a complete evaluation.

There are two reasons for this,Jumy Fadugba, MD, FAAAAI, study author, chief of section of allergy and immunology at Penn Medicine, andassociate professor of clinical medicine and fellowship program director of allergy and immunology at the University of Pennsylvania Perelman School of Medicine, told Healio.

People with a penicillin allergy label have significantly worse clinical outcomes than those who don’t have one, Fadugba continued. Additionally, patients who use more broad-spectrum antibiotics have more microbial antibiotic resistance, which is a rising public health problem, and gut infection such as Clostridium difficile, she said.

Preventing Allergies In The Unborn Infant

As far as the unborn babys health is concerned, the pregnancy and treatment thereof can have profound impact on the foetuss health and predisposition to allergy. We know that if the mother has allergies there is a 30% chance of her baby having an allergy. If both parents have allergy, the babys risk increases to 60% and if both parents have the same allergy for example asthma, then the risk of allergy in the unborn baby reaches 80%. Maternal smoking during pregnancy and the avoidance of allergy provoking foods in her diet in the last few months of pregnancy may encourage allergic diseases to develop in the baby. However the evidence is still lacking that diet in pregnancy makes any difference. A hypoallergenic diet may also be more disadvantageous causing possible maternal and foetal malnutrition rather than advantageous for allergy prophylaxis. American and European guidelines thus recommend a normal diet without any exclusions during pregnancy and whilst breastfeeding with the exception of adding probiotics. However if the baby does have an established allergy , then the nursing mother should avoid these foods in her diet as traces do pass into the breast milk.

Use of a very broad antibiotic means you are more likely to become infected with a bug that is resistant to many different drugs, he says. And you are potentially creating a situation where a bad bug can evade antibiotics, proliferate, and become more prevalent in the community.

Two YNHH physicians have developed an initiative to test and identify pregnant women who report penicillin allergies in order to verify or clear the reported allergy.

Yale physicians developed an allergy testing program for pregnant women that aims to verify penicillin allergies, keeping patients from taking unnecessary stronger antibiotics and helping to curb drug resistance.

Jason Kwah, an immunologist and assistant professor at the Yale School of Medicine, and Moeun Son, an assistant professor of obstetrics, gynecology and reproductive science at YSM, founded the Allergy, Asthma and Immunology Care in Pregnant Mothers Program in September 2020. For the past two years, they have been conducting allergy testing on 235 expectant mothers and have found that all but 2 of those women were cleared of their penicillin allergy after testing.

Penicillins and cephalosporins are beta-lactam antibiotics, which are the most commonly used antibiotics that are considered first-line for many infections. How To Avoid Allergies While Pregnant

Reducing The Risk Of Allergies In Childhood

Plan breast-feeding carefully! The best scenario is at least 4 months exclusive breast feeding with no top-up formula feeds in a mother who takes probiotic supplementation. Maternal avoidance of allergenic foods during pregnancy and whilst breastfeeding does not seem to have any impact on the infants allergic status. Allergic sensitisation tends to occur very early in life through minor traces of allergens being breathed in the air and contact with the skin when handling the infant after touching foods.
