Diagnosis Of Cows Milk Allergy

Lactose intolerance – causes, symptoms, diagnosis, treatment & pathology

If you or your child have allergic symptoms, visit your family doctor who will ask some questions about your reactions. You can also discuss your record of your symptoms. To diagnose your allergy, your doctor may refer you to a clinical immunology/allergy specialist. If your symptoms appear rapidly after eating or drinking milk or dairy products, the allergy may be easier to diagnose, whereas symptoms that take longer to appear make diagnosis more difficult.Allergists can test for allergies using a number of methods depending on the type of potential allergy. To test for an allergy, the allergist may:

  • do a skin prick test
  • do a blood test
  • ask you to temporarily avoid all milk or products containing milk , then follow up with the introduction of milk back into your diet under strict medical supervision.

A number of methods claim to test for allergies but have not been medically or scientifically proven. They can be costly and could lead to dangerous avoidance of certain foods. The Australasian Society of Clinical Immunology and Allergy recommends that you do not use certain methods to have potential allergies tested, including:

  • cytotoxic food testing

Always speak with your doctor if you are thinking of using a complementary medicine or therapy for allergies.

What Is A Dairy Allergy

A dairy allergy is an immunological reaction to the proteins in milk . When an individual with a milk allergy consumes dairy of any kind, the body triggers an immune response to fight off what it sees as a harmful substance. Individuals can experience an array of symptoms that are a result of this immune response.

How Is Milk Allergy Treated

The best way to manage all types of milk allergies and milk sensitivities is to avoid consuming dairy products. While there is no definitive treatment for milk allergy, some of the following measures may be taken to limit reactions:

  • Infants who develop milk allergies way before starting on solid foods may be prescribed hypoallergenic infant formula.
  • For a life-threatening allergic reaction, treatment includes administering injectable epinephrine, oral or injectable antihistamines, or oral or injectable corticosteroids.
  • Treatment of food protein-induced enterocolitis syndrome includes administering intravenous hydration and intravenous corticosteroids.

How Do You Avoid Exposure If You Have A Milk Allergy

If you have a milk allergy, strict avoidance of milk is the only way to prevent an allergic reaction. The Food and Drug Administration requires food manufacturers to list common food allergens on food labels in plain terms to make it easier to identify the food allergens. Food labels must clearly list eight allergens which account for almost 90% of all food allergies: cows milk, soy, wheat, egg, peanut, tree nuts, fish, and shellfish.

The common allergens are listed either within the ingredient list or after the list. For example, if a product contains casein, a milk protein, the product’s label should list the term milk either after the term casein, or state contains milk after the list of ingredients. The FDA currently does not require manufacturers to state if the food was processed in a facility that also processes the 8 common food allergens.

Anyone allergic to milk should avoid the following ingredients/foods:

  • Milk: in all forms, including condensed, dry, evaporated, and powdered milk, and milk from mammals .
  • Casein and casein hydrolysates.
  • Lactalbumin, lactalbumin phosphate, lactoglobulin, lactoferrin and lactulose.
  • Butter: including butter, butter fat, butter oil, artificial butter flavor.
  • Cream, half & half, and ice cream.
  • Cottage cheese and curds.
  • Sour cream, sour milk.

Many children with a milk allergy can tolerate highly heated baked forms of milk, such as muffins and cake. Discuss this with your doctor for specific dietary recommendations.

If you are lactose intolerant you can still ingest milk. There are several over-the-counter remedies, like lactaid, that provide lactase enzymes. There are also specially treated dairy products, with added enzymes, and you may also have success with yogurt and other fermented milk products.

If you have a milk allergy you should not ingest milk, even if your reaction to milk is relatively mild, according to Food Allergy Research and Education 2. The reason is that, while your reaction may be mild now, it can become worse over time â leaving you at great risk for anaphylaxis. If you have a milk allergy it would be better to consume milk substitutes like soy or rice products.

How Common Is Cma

CMA is a common childhood allergy, affecting 23% of children before the age of 3 years. Babies who have close family members with allergic conditions such as eczema and asthma are more likely to have CMA.CMA is less likely in babies who are only breastfed, as their only source of cows’ milk protein comes through their mother’s diet. This is associated with a lower risk of allergy.

Treatment For Cows Milk Allergy

If you or your child have been diagnosed with milk allergy, treatment involves total avoidance of milk or products containing milk. This can be difficult to achieve.

The Australasian Society of Clinical Immunology and Allergy advises that children who are allergic to cows milk are often allergic to goats milk, and so substitution with a milk of animal origin may not be possible. To avoid milk and milk products, read food labels and avoid foods that contain milk or milk products such as:

ASCIA has more information about dietary avoidance of cows milk for food allergy.Eliminate milk and milk products from your baby or young childs diet only under strict medical supervision. Your doctor will need to advise on replacement sources of calcium and protein, which are especially important nutrients for children. Replacement formulas for babies also needs to be carefully supervised. For example, some children with cows milk allergy will not be able to tolerate soy milk products.

Symptoms Of A Milk Allergy Reaction

Symptoms can range from mild to severe. As with any significant food allergy, some reactions can even be life-threatening. Some of the most common symptoms of milk allergy reaction include:

  • Confusion and/or dizziness
  • Weak, rapid pulse

Those with a diagnosed dairy allergy are advised to carry two epinephrine auto-injectors at all times.

How Is Cows Milk Allergy Managed

Food Allergy 101: Manage Milk Allergies | Milk Allergy Symptom

If you or your child are allergic to cows milk, you will need to remove all cows milk from your diet. This isnt easy because milk is an ingredient in many foods, such as baked goods, cereals, chocolate, sweets, sausages, salad dressing and bread. It can also be found in some milk, cream and butter substitutes labelled non-dairy.

Follow your doctors instructions and check food labels very carefully. Watch out for other words used to describe milk on food labels, such as butter, buttermilk, cream, curd, ghee, milk, cheese, dairy, milk solids, whey, yoghurt, casein and caseinates.

Some labels warn that the food “may contain traces of milk”. This usually means the food is made in a facility that also makes a food containing milk. Talk to your doctor about whether it is safe to eat those foods.

You may also need to avoid milk from other animals, such as goats and sheep milk 90% of children will react to these milks if they have cows milk allergy.

If you or your child has a cows milk allergy, its important to have a personal action plan to manage an allergic reaction. Your doctor may prescribe an adrenaline autoinjector. For more information on anaphylaxis, including setting up a personal action plan, go to www.allergy.org.au.

How Is Cows Milk Allergy Diagnosed

The diagnosis of cow’s milk allergy is often obvious when symptoms occur within minutes of exposure. Skin prick allergen tests from your doctor can confirm the diagnosis. When symptoms are delayed, cow’s milk allergy can be harder to diagnose. Not every child who has a positive allergy test will develop symptoms when exposed to milk.

The Australasian Society of Clinical Immunology and Allergy recommends you should speak to your doctor or specialist about the benefits and safety of allergen immunotherapy or before attempting any allergy testing or treatment.

How Do You Test For A Dairy Intolerance

YorkTest, Europes leading provider in food intolerance testing, has been providing IgG food tests, which include dairy-specific IgG antibody reactions, for 40 years.

It is estimated that around 45% of the population suffer from food intolerances such as a dairy intolerance. YorkTest has found that the average person with positive food IgG reactions, reacts to 4 to 5 different ingredients. Our Premium Food Intolerance Test, for example, can assess the level of anti-dairy antibodies in the blood, reacting to ingredients such as cows milk, sheeps milk and goats milk. This specific testing can help narrow down your triggers and provide a more detailed look at your gut health for example, you may find you react to cows milk but can tolerate other animal milks. Therefore, its always worthwhile to consider an IgG food intolerance test before jumping into an elimination diet by yourself.

Please note that YorkTests programmes do not measure the levels of lactase, and therefore do not diagnose lactose intolerance. If the food intolerance test results indicate that you may have a dairy intolerance, it does not mean that you are lactose intolerant, although you may wish to consult your doctor for further tests, as you may have both.

If Your Child Has An Allergic Reaction

If your child has symptoms of an allergic reaction, follow the food allergy action plan your doctor gave you.

If your child has symptoms of a serious reaction :

  • Give the epinephrine auto-injector right away. Every second counts in an allergic reaction.
  • Then, or take your child to the emergency room. Your child needs to be under medical supervision because, even if the worst seems to have passed, a second wave of serious symptoms can happen.

Living With Dairy Allergy

Signs & Symptoms of Dairy Allergy

If you have dairy allergy, youâll need to avoid all dairy foods and other foods that contain dairy products.

Staying safe means reading food labels to see if milk or ingredients containing milk are included. Milk proteins are found in many foods you wouldnât expect. Some canned tuna, energy drinks and even chewing gum contain them. And donât eat lactose-reduced foods if you have dairy allergy. They still contain the milk proteins that can cause allergic reactions.

How Is A Milk Allergy In Infants Diagnosed

If your baby has symptoms of either a milk allergy or milk intolerance, see your pediatrician. He or she will probably check for both with a physical exam, an examination of your babys stool, a discussion your family’s history of allergies and/or a skin-prick test.

If your doc thinks your baby may have a milk allergy or intolerance, then youll probably be asked to eliminate milk from your diet or switch to a different formula to see whether your newborns symptoms improve.

After your baby has been cows-milk-free for about a week, the doctor may have you reintroduce cows milk to see whether your little one has a reaction to it.

Signs Of Dairy Intolerance In Babies

Colic is considered normal and eczema is usually treated with medical creams. Yet both of these are possible signs that formula milk or something in a mothers breastmilk is causing problems.

In my experience, most health professionals are concerned with life-threatening food reactions only. I suspect that this is because a diet without cows milk is considered inadequate, and alternative formula milks are somewhat lacking and only to be used as a last resort.

There is also hope that as a baby gets older, their digestive system strengthens, immune system normalises and the problem will go away. Dietitians will try to train the body to accept dairy products by putting them back into the diet a little at a time. Sometimes this is successful, but other times it isnt. It appears that all has gone well, but the child develops other health issues that are considered unrelated to dairy when actually they are.

The issue of dairy intolerance is easier to address if you are breastfeeding your baby. You have control over what you eat and therefore what goes into your babys milk.

Its not always easy to make big dietary changes when you are post-natal and sleep-deprived, but its worth it. However, you do need to be strict about elimination. Cutting out milk and leaving in cheese or just having milk in cups of tea may not be enough. You need to cut it all out to find out for sure.

Rapid Onset Allergic Reactions To Cows Milk And Other Dairy Foods

Rapid onset allergic reactions usually occur within 15 minutes and sometimes up to two hours after consuming cow’s milk or other dairy foods. Symptoms include one of more of the following:

  • Mild or moderate allergic reactions such as swelling of lips, face, eyes, hives or welts on the skin, stomach pain, vomiting and diarrhoea.
  • Severe allergic reactions include noisy breathing or wheeze, swelling or tightness in throat, young children may be pale and floppy.

Anaphylaxis should always be treated as a medical emergency, requiring immediate treatment with adrenaline and calling for an ambulance.

Alternative Milks In Children Over One Year Of Age

6 signs you might be lactose intolerant
  • Soy milk, calcium enriched rice, oat or nut milks may be recommended by your doctor and/or dietitian, depending on your childs condition.
  • It is important to check if they contain adequate levels of protein and fat for young children for growth.
  • Alternative milks enriched with calcium must contain around 120mg/100mL to be a suitable cows milk replacement.
  • Children with multiple food allergies may need to continue on specialised formula to meet their nutritional requirements.
  • Babies should be reviewed by a dietitian at around 12 months of age so the need for formula can be considered.

Alternative Milks For Babies

Soy protein formula

  • Tolerated by most babies with cow’s milk allergy.
  • Unsuitable for babies allergic to soy.
  • Usually only recommended in babies over six months old.

Cows milk based extensively hydrolysed formula

  • EHF has been treated with enzymes to break down most of the cows milk proteins and it is usually the formula of first choice in cows milk allergic babies.
  • EHF is not suitable for babies who have had anaphylaxis to cows milk.
  • Some EHF brands are available without prescription.
  • An amino acid based formula is usually prescribed if a baby reacts to EHF.
  • Partially hydrolysed formula is not a suitable formula for babies with cows milk allergy as enough allergenic protein is usually present to trigger an allergic reaction.

Rice protein basedformula

  • May be used as an alternative formula to EHF or soy protein formula and continued or changed based on specialist advice.
  • Available without prescription.
  • Should not be used in babies with food protein induced enterocolitis syndrome to rice.

Amino acid based formula

  • AAF is necessary in around one in ten babies with cow’s milk allergy.
  • AAF is usually prescribed when an EHF or soy protein formula is not tolerated.
  • AAF is tolerated by most babies with cow’s milk and soy allergies.

What Are The Symptoms Of Cows Milk Allergy

The symptoms of allergy to cow’s milk can range from mild to severe.

Mild to moderate symptoms include:

  • swelling of the lips, face and eyes

Anaphylaxis is the most severe allergic reaction and requires immediate treatment. Symptoms include:

  • noising breathing or wheeze
  • swelling or tightness in the throat
  • loss of consciousness and floppiness in babies or young children

If you think someone is having an allergic reaction, seek medical advice urgently as symptoms can worsen rapidly. If breathing is affected, call triple zero .

Risk Factors For Lactose Intolerance

Most humans usually stop producing lactase with age, and only a few can continue to digest lactose throughout their lives (

  • more than 50% of people in South America, Africa, and Asia
  • up to 100% of people in Asian countries
  • children of African, Asian, or Hispanic descent, whose symptoms may begin between 23 years of age
  • children of European and American descent, whose symptoms may appear between 56 years of age or during adolescence

On the contrary, the frequency of the lactase-persistence trait is higher in northern European populations .

Milk Allergy Vs Lactose Intolerance

Milk Allergy And Lactose Intolerance Symptoms, Causes And Prevention ...

The difference between a milk allergy and lactose intolerance is the involvement of the immune system. When a child has a milk allergy, the bodys immune system creates IgE antibodies to milk protein which lead to release of histamine and other chemicals that cause symptoms typical of allergic reactions

Lactose intolerance, on the other hand, involves more of the GI system. Those with lactose intolerance lack an enzyme that breaks down the sugars in milk and dairy products. This causes GI symptoms that may include nausea, cramps, gas, bloating and diarrhea.

Is It An Allergy

Lactose intolerance is not the same as a milk or dairy allergy. Food allergies are caused by your immune system reacting to a certain type of food. This causes symptoms such as a rash, wheezing and itching.

If you’re allergic to something, even a tiny particle can be enough to trigger a reaction, while most people with lactose intolerance can still consume small amounts of lactose without experiencing any problems, although this varies from person to person.
