Could Painkillers Prevent Recurring Urinary Tract Infections

An estimated 50% of all women will experience a urinary tract infection at some point in their lives, and 20-40% of these women will have recurring infections, putting them at risk for kidney and bloodstream infections. But now, new research suggests over-the-counter painkillers could help prevent such infections from occurring.

Investigators from Washington University School of Medicine in St. Louis, MO, presented their findings at the annual meeting of the American Society for Microbiology.

They found that, by inhibiting an immune protein that causes inflammation called COX-2 they were able to eliminate repeat urinary tract infections in mice. The team explains that COX-2 can be blocked by non-steroidal anti-inflammatory drugs , which includes ibuprofen.

According to the Centers for Disease Control and Prevention , urinary tract infections are responsible for around 4 million ambulatory-care visits every year in the US.

Most UTIs are caused by the bacterium Escherichia coli, which lives in the bowel. While men also get UTIs, women are especially prone to them because their urethra is shorter which allows bacteria better access to the bladder and a womans urethral opening is in proximity to bacteria sources from the anus and vagina.

Each time a woman has a UTI, her chances of having recurrent infections increases, and severe infections with can even be fatal.

    What Are The Implications

There is need to reduce antibiotic use to tackle growing antibiotic resistance. But this study doesnt indicate that prescribing anti-inflammatories for urinary tract infection is the answer.

Women who have several symptoms are almost certain to have a bacterial infection. Giving anti-inflammatories may reduce antibiotic use, but symptom resolution is less likely, and there is a higher risk that infection will ascend to the kidneys.

As the authors note, they chose an antibiotic with known effectiveness in their region. In countries and settings with higher levels of antibiotic resistance, the difference between antibiotics and NSAIDs may be less marked.

Reduced Side Effect On The Gastrointestinal System

So, the question now is if you need to take the COXIBs in place of an NSAID like Ibuprofen, how will it impact the body? Stomach bleeding is a key concern for most that are not able to take Ibuprofen.

Though the side effects are not completely absent over the longer term, there are fewer instances of adverse impact. Patients who have been taking Celecoxib show significantly better gastric conditions. That also implies COXIBs impact the stomach region in a relatively mild way.

Moreover, despite several studies on the issue, scientists have not found a direct correlation. Instances like GI bleed or tearing of the inner membrane of the stomach are common in the case of Ibuprofen overdose. However, with Celecoxib, this is not a great source of concern.

Studies also indicate when you use medicines like Rofecoxib and Celecoxib instead of Ibuprofen, it does not affect the permeability of the small intestine as drastically. If someone is taking Ibuprofen for an extended period, it increases the permeability to a large extent.

That way, the COXIBs offer substantial benefits over the longer term. As a result, the threat of significant damage over the long-term comes down too.

Asthmatics With Nasal Polyps Should Avoid Nsaids Others Use With Caution

NSAID-induced bronchospasm should be suspected in any patient whose asthma control worsens on initiation of a NSAID. Patients with a history of asthma should be warned of this reaction and to seek medical help if symptoms worsen on initiation of a NSAID.

NSAIDs should be used with caution in the presence of asthma and avoided in asthmatics with nasal polyps. As it is difficult to identify at riskasthmatics, it would seem prudent to prescribe paracetamol instead of aspirin unless there are any specific contra-indications.

Patients should be reminded to read labels of over-the-counter medicines as some, such as cough/cold preparations, may contain aspirin. Ibuprofen, diclofenac and other NSAIDs are also available over-the-counter.

    How To Prevent Treat Ulcers

    Ibuprofen: Important Warnings and Precautions

To lower your risk of ulcers, follow the recommended-use instructions on products containing NSAIDs, Abdi said. These typically note that the medication shouldnt be taken for longer than 10 to 14 days. The longer you take it and the higher dose that you take it outside of the labeled use, youre at greater risk for developing complications, she said.

Switching to a lower-risk pain reliever, such as Tylenol, or acetaminophen, may also be helpful, experts said. But although acetaminophen treats pain, it isnt an anti-inflammatory, and taking too much can cause liver damage.

Experts recommended that people who use NSAIDs to help manage chronic pain talk with their health-care provider, rather than self-medicate. Keep in mind that there isnt a specific dosage or use period that is considered universally safe, Stevoff said.

Everybodys different, he said. For some people, they can take massive doses of these things and never have a problem. Other people can take relatively small amounts and get an ulcer right away.

If your treatment requires taking NSAIDs over a long period, your doctor may also prescribe a common acid-reducing medication known as proton pump inhibitors, or PPIs, experts said. Some PPIs, such as Prilosec and Nexium, are over-the-counter medicines.

As I have learned, NSAIDs have side effects and ignoring their potential impact isnt a good idea, she . I am exhibit A.

    Ibuprofen Helps Relieve Allergic Rhinitis

    Pharmacy Times

Researchers have found that adding low-dose ibuprofen to the usual allergy relief treatment of chlorpheniramine and pseudoephedrine improves relief from seasonal allergic rhinitis. The findings from the 7-day trial were presented recently at the American College of Allergy, Asthma and Immunology Annual Meeting.

For the study, the investigators randomized patients to a placebo or to treatment in a double-blind, double-dummy, parallel-group fashion. The participants had at least a 2-year history of seasonal allergic rhinitis. The first group received a triple combination of ibuprofen, pseudoephedrine, and chlorpheniramine. In the first arm of the triple combination group, participants received 200 mg ibuprofen, 30 mg pseudoephedrine, and 2 mg chlorpheniramine. Participants in the second arm received 400 mg ibuprofen, 60 mg pseudoephedrine, and 4 mg chlorpheniramine. The second group received 30 mg pseudoephedrine and 2 mg chlorpheniramine without ibuprofen. The third group received a placebo.
