How To Take And Store

According to the manufacturer:

  • For asthma and allergies: Take Singulair once a day in the evening.
  • For exercise-induced bronchoconstriction: Take Singulair at least two hours before a physical activity that may trigger symptoms.

Singulair can be taken with or without food.

Oral granules for children must be given within 15 minutes of opening the package, so don’t open it until it’s time to give it to your child, which you can do in one of three ways:

  • Spoon it right into their mouth
  • Dissolve it in a teaspoon of cold or room-temperature formula or breastmilk ONLY
  • Mix it with a spoonful of applesauce, mashed carrots, rice, or ice cream

Throw away any of the mixture that’s left.

You should store Singulair:

  • Suicidal thoughts, behaviors, and attempts
  • Tics
  • Tremor

While individuals with and without a history of such concerns may experience any of these while on Singulair, it is especially important that those who have had such issues in the past inform their healthcare providers before starting the drug.

If you notice any of these side effects while on Singulair, talk with your prescribing healthcare provider immediately. The FDA recommends that your healthcare provider consider discontinuing your prescription should that occur.

Fda Requires Stronger Warning About Risk Of Neuropsychiatric Events Associated With Asthma And Allergy Medication Singulair And Generic Montelukast

The U.S. Food and Drug Administration today announced that it is requiring a boxed warning the agencys most prominent warning for montelukast to strengthen an existing warning about the risk of neuropsychiatric events associated with the drug, which is used to treat asthma and allergic rhinitis . The boxed warning advises health care providers to avoid prescribing montelukast for patients with mild symptoms, particularly those with allergic rhinitis.

As noted in a new Drug Safety Communication issued today, the warning follows the FDAs review of available data regarding continued reports of neuropsychiatric events with montelukast, such as agitation, depression, sleeping problems, and suicidal thoughts and actions. The Drug Safety Communication includes recommendations for health care professionals and patients and a summary of the data that led to these warnings.

The FDA updated the product labeling in 2008 to include information about neuropsychiatric events reported with use of montelukast. In response to continued reports of suicide and other adverse events, the FDA evaluated available data regarding the risk of neuropsychiatric events, including reports submitted through the FDA Adverse Event Reporting System and observational studies in the published literature. The FDA also conducted an observational study using data in the Sentinel Distributed Database and presented the findings at an FDA advisory committee meeting in 2019.

How Does This Medication Work What Will It Do For Me

Montelukast belongs to a group of medications known as leukotriene receptor antagonists. It is used to prevent and manage asthma symptoms and to relieve the symptoms of seasonal allergies, also known as seasonal allergic rhinitis or hay fever. It may also be used to treat asthma symptoms caused by exercise . It works by blocking the effects of leukotrienes, a substance produced by the body in response to certain “triggers” that cause narrowing and swelling of airways in the lungs. It may be taken alone or in addition to other asthma medications. Montelukast usually starts to work within one day.

When used to prevent and manage asthma, it is important to understand that montelukast should not be used to relieve an asthma attack once it has started. People with asthma should always have their fast-acting asthma relief medication on hand to treat an asthma attack.

Your doctor may have suggested this medication for conditions other than the ones listed in these drug information articles. As well, some forms of this medication may not be used for all of the conditions discussed here. How To Take Singulair

Use Singulair exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended.

Follow all directions on your prescription label and read all medication guides or instruction sheets. Use the medicine exactly as directed.

Montelukast is not a fast-acting rescue medicine for asthma attacks. Seek medical attention if your breathing problems get worse quickly, or if you think your medications are not working.

Montelukast is usually taken once every evening, with or without food.

For exercise-induced bronchoconstriction, take a single dose at least 2 hours before exercise, and do not take another dose for at least 24 hours.

Swallow the regular tablet whole, with a glass of water.

You must chew the chewable tablet before you swallow it.

Place the oral granules directly in your mouth and swallow, or mix them with applesauce, mashed carrots, rice, or ice cream. The granules may also be mixed with baby formula or breast milk. Do not use any other type of liquid. Use the mixture within 15 minutes. Do not save for later use.

If you also use an oral steroid medication, you should not stop using it suddenly. Follow your doctor’s instructions about tapering your dose.

Do not change your dose or stop using asthma medication without your doctor’s advice.

Store at room temperature away from moisture and heat. Do not open a packet of oral granules until you are ready to use the medicine.

Cautions With Other Medicines

Tell your doctor if you are taking any of the following medicines:

  • epilepsy medicines such as phenobarbital and phenytoin
  • rifampicin
  • gemfibrozil

It’s usually safe to take everyday painkillers with montelukast. However, do not take non-steroidal anti-inflammatory drugs such as aspirin or ibuprofen if they have ever made your asthma symptoms worse.

What Safety Concern Is Fda Announcing

The U.S. Food and Drug Administration is strengthening existing warnings about serious behavior and mood-related changes with montelukast , which is a prescription medicine for asthma and allergy.

We are taking this action after a review of available information led us to reevaluate the benefits and risks of montelukast use. Montelukast prescribing information already includes warnings about mental health side effects, including suicidal thoughts or actions however, many health care professionals and patients/caregivers are not aware of the risk. We decided a stronger warning is needed after conducting an extensive review of available information and convening a panel of outside experts, and therefore determined that a Boxed Warning was appropriate.

Because of the risk of mental health side effects, the benefits of montelukast may not outweigh the risks in some patients, particularly when the symptoms of disease may be mild and adequately treated with other medicines. For allergic rhinitis, also known as hay fever, we have determined that montelukast should be reserved for those who are not treated effectively with or cannot tolerate other allergy medicines. For patients with asthma, we recommend that health care professionals consider the benefits and risks of mental health side effects before prescribing montelukast.

Warnings the drug could cause depression, aggression, and suicide began appearing in the package insert for Singulair in 2008, a decade after the drug was put on the market, but the risks were not generally advertised.

According to the FDA, side effects for Singulair and its generics including suicidal thoughts and actions, agitation, aggressive behavior or hostility, attention problems, bad or vivid dreams, depression, disorientation, or confusion, feeling anxious, hallucinations including seeing or hearing things that are not there, irritability, memory problems, obsessive-compulsive symptoms, restlessness, sleepwalking, stuttering, tremor or shakiness, trouble sleeping and uncontrolled muscle movements.

In September of 2019, the FDA held a public meeting titled ‘Neuropsychiatric Events with the Use of Montelukast in Pediatric Patients.’ Montelukast is the generic of Singulair.

The FDA provided data showing more than 10,000 nervous system and psychotic events attributed to Singulair had been reported to the agency through their ‘FDA Adverse Event Reporting System.” FAERS is a database where the public and health care professionals can report problems with a medication. Some health experts at the meeting believe the thousands of reports represent only a fraction of the actual number of problems experienced by patients.

Additional side effects include an increased tendency to bleed, low blood platelet count, allergic reactions, dizziness, drowsiness, numbness or pins and needles, seizures, and heart palpitations. In some cases, people may have pain in their joints or muscles, muscle cramps, tiredness, and swelling, and in children, bed-wetting may occur.
